Nikoran contains Nicorandil that relaxes coronary vascular smooth muscles

and helps dilate coronary arteries and arterioles. It, thus improves coronary blood flow by lowering coronary arterial resistance. It is useful in treating chest pain (Angina).

 

The actions of Nikoran result in ‘balanced’ arterial and venous dilatation. Another benefit of Nikoran is, that it is not associated with tolerance or rebound angina. It results in balanced offloading of the ventricles (Chambers of heart) by reducing

preload (Preload is the initial stretching of heart muscle cells prior to contraction) and afterload (Afterload is the force against which the heart has to contract to pump the blood).

Angina pectoris

 

Angina pectoris is the medical term for chest pain or discomfort due to coronary heart disease.  It occurs when the heart muscle doesn’t get as much blood as it needs. This usually happens because one or more of the heart’s arteries is narrowed or blocked, also called ischemia.

 

Angina usually causes uncomfortable pressure, fullness, squeezing or pain in the centre of the chest. The pain or discomfort occurs when the heart must work harder, usually during physical exertion. Doesn’t come as a surprise, and episodes of pain tend to be alike. It usually lasts a short time (5 minutes or less)

Adverse Reactions/ Side Effects

Headache (usually transitory), flushing, dizziness, nausea, vomiting and weakness. Hypotension and reflex tachycardia at high doses.

Warnings and Precautions

Hypovolaemia, low systolic BP, acute pulmonary oedema, pregnancy. May impair ability to drive or operate machinery.

Contraindications

Cardiogenic shock; hypotension; left ventricular failure with low filling pressure; lactation.

Drug Interactions

Hypotensive interaction may occur with alcohol, TCAs, antihypertensives and other vasodilators.

 

Potentially Fatal: Enhanced hypotensive effect with sildenafil and other phosphodiesterase type-5 inhibitors.

Mechanism of Action

Nicorandil dilates arterioles and large coronary arteries by opening the potassium channels, and stimulates guanylate cyclase causing venous vasodilatation. It therefore reduces preload and afterload, and improves coronary blood flow.
